Assumption of Risk, Release of Liability and Waiver
Activity and known risks. Volleyball is an active, physical sport. The known inherent risks of volleyball clinics include sprains and strains; jammed, dislocated or broken fingers, wrists, ankles and other bones; knee injuries including torn ligaments; being struck by a ball, another player, the net, poles or equipment; falls and collisions on a hard gym floor; concussions and other head injuries; overheating, dehydration and exhaustion; and, in rare cases, serious injury or death.
Assumption of risk. I understand these risks. I choose to have the player named in this form take part in Bitterroot Volleyball Club (BVC) clinics, and I accept the inherent risks of volleyball, whether known or unknown, as provided by Montana Code 27-1-753.
Release. In exchange for the player being allowed to take part, I release, waive and agree not to sue Bitterroot Volleyball Club, its coaches, volunteers and organizers, and the owners and operators of the facilities used, for any injury, loss or damage to the player arising from participation in BVC clinics, including injury caused by the ordinary negligence of any of them. This release does not cover gross negligence or intentional misconduct.
Medical care. If I cannot be reached in an emergency, I authorize BVC coaches to get emergency medical care for the player, and I accept responsibility for the cost of that care. I confirm the player is physically able to take part in volleyball activity.
Signer. I am the player's parent or legal guardian, or I am the player and I am 18 or older. I have read this release, I understand it, and I am signing it voluntarily. My typed name below is my signature.
